(employee + spouses + dependents) for group medical

2004 Individual Sales Bonus
All Individual* broker sales receive a monthly
bonus based on the monthly sales production. This is
how it works: each month, the number of newly written
paid contracts is calculated and a bonus is paid based
on the schedule below. Once the next month arrives,
the calculation starts all over again.
This bonus is in addition to commissions. To count toward
the bonus, members must have paid a minimum of the first
month's rates. The bonus is not accumulative if more
than one level is achieved. See the example below.
