06/02/2008  Regence MedAdvantage enrollment applications updated June 1

Due to Centers for Medicare & Medicaid (CMS) regulations, we are changing the verbiage on our Regence MedAdvantage enrollment applications. Effective June 1, 2008, all current blank applications must be replaced with the new version. You can get new applications from your Regence Sales contact or from our Web site.

The changes to the enrollment applications are minor:
  • We no longer require a member to provide authorized representative information (such as power of attorney) at the time of enrollment.
  • Deletion of question #5: We no longer are required to ask if an individual has current or past credible coverage.
Older versions of the application used after June 1, 2008, will be out of compliance with CMS regulations. This does not affect Medigap or stand-alone part D applications.
