Guidelines to waiving the probationary period for key employees
Effective Jan. 1, 2008, we changed the guidelines for waiving a key employee's probationary period when the waiver is a condition of employment.
Each request must be accompanied by the enrollment application in order for the underwriter to review it. To make this process easier for you and your groups, we have created a key employee waiver form that must be completed and returned along with the application.
