• Provides specialized, targeted attention and support to patients with complex health issues
• Combines the core functionality of Regence Utilization Management, Regence Case Management, Regence Disease Management, Special Beginnings^® and Health Coach into a single blended program, designed to better support the health needs of your patients
• Utilizes predictive modeling tools that proactively identify gaps in care, incorporating evidence-based clinical care plans and interventions in support of your directed treatment plan
  

Patient support is delivered by a multi-disciplined Personal Care Team that includes a range of specialties, including:

• Pharmacist
• Medical Director
• Disease Manager
• Clinical Resource Nurse
• Behavioral Health Clinician
• Care Advocate (Health Coach)
• Case Management Specialists
• Pediatric Case Manager
• Complex Case Manager
• Transplant Case Manager
• High Risk Maternity Nurse

ICM is appropriate for:

• Patients of all ages
• Patients with complex or catastrophic medical condition(s) or event(s), both acute and chronic
• Patients identified as needing additional support and guidance through medical challenges

ICM supports physicians with:

• Earlier identification and outreach to patients with gaps in care
• Evidence based treatment plans that encourage condition-appropriate behaviors, such as proper medication use and regular physician follow-up
