Estimates
Due to popular demand, we have modified our policy
for estimating only major services. Many of you commented
that our limitation to major services was a hardship
for your patients.
Therefore, effective April 1, 2001, pre-treatment plans
totaling $300 and above will be accepted for a courtesy
estimate.
To expedite processing, please submit the appropriate
documentation, such as x-rays, periodontal charting
or narratives, with the original pre-treatment plan.
Crown build-ups
Use codes D2950 or 02950 when building up a tooth for
adequate retention of a crown. Use codes D2140 through
D2388 when billing for amalgam or composite restorations,
as these codes are not appropriate for buildup.
Replacements for restorative
and prosthetic procedures
When billing for replacement of crowns, onlays, bridges,
partials or dentures, be sure to include:
- Dated pre-treatment x-ray.
- Date of initial placement.
- Narrative indicating reason for replacement.
Thank you for your cooperation as these steps will help
avoid delays in processing due to inadequate information.
Multiple periapicals
on the same day
When billing for multiple periapical x-rays taken on
the same date of service, please list the first periapical
with code D0220 or 00220, and all subsequent periapicals
with D0230 or 00230 and the number of additional periapicals.
X-rays submitted with
claims
First of all, we greatly appreciate those of you who
take the time to submit an x-ray with a claim.
However, x-rays are separated and reassembled in our
mail room for imaging purposes. Unfortunately, an x-ray
can be separated from a claim during this process. If
the x-ray and/or envelope is not marked with identification,
it is almost impossible for us to re-match the x-ray
with its claim. Most important, we are unable to return
the x-ray to you. Therefore, it is important that you
label each x-ray envelope with:
- Patients name.
- Date of x-ray.
- Dentists name and address.
