feature flexible options; allowing employers
to choose copayment, coinsurance and deductible
amounts. Members can seek services from any provider.

Choice Account and convert to dollars that

including deductibles and coinsurance.

front of the member card. Coinsurance, deductible
and network information is not included on the

Activate Member Choice Fund
payments
When an Activate member elects to use his
or her Member Choice Fund to pay medical
deductibles or coinsurance, the payment
will be noted with a new explanation code
of ZMO, Member Choice Account payment.
